Church Agreement
For the Student Pastor or Group Leader to complete and certify all volunteers have passed a background check and have completed Lifeway’s Child Protection Training course (below).
Waiver and Release Form
Each person attending Student Life Camp must complete an online Waiver and Release form, including all adult leaders. Group Leaders will receive a copy of the completed forms to their email address. Be sure to print the How-To documents for parents below.
*NEW* Child Protection Training
Each adult attending Student Life Camp must complete Lifeway’s Child Protection Training course. Group Leaders will verify all adults have completed this course by signing the Church Agreement (above). This course is the ONLY approved curriculum for Student Life Camp.
Certificate of Insurance
Lifeway is asking for a Certificate of Insurance (COI) for a Commercial General Liability policy from each church attending a Lifeway camp for 2023. This type of liability insurance covers the church from any claims made against your group due to your negligence while attending camp. Southeastern University
Southeastern University Swim Permission
In addition to the Student Life Waiver and Release form, Southeastern University requires this Minor Swim Permission Waiver for all minors wanting to use the swimming pool. Please advise parents to enter “Student Life Camp” as the camp name.
YMCA Estes Park
YMCA Chaperone Agreement
Every adult leader in your group (18 and older) will need read and sign this form. Please print and bring these forms to check in with you for YMCA staff to collect.
