Church Agreement
This form certifies all volunteers have passed a background check and have completed Lifeway’s Child Protection Training course (below).
This form will become available to sign via your Participant List link once your final Participant List has been submitted (no later than 14 days before camp). The form will auto-populate the adult leaders’ names that were included on your list (no more typing line by line!). Additional names can be added (even if they are not included on your Participant List) if you need to add backup names in the event you need to replace someone before camp.
Waiver and Release Form
Each person attending Student Life Camp must complete an online Waiver and Release form, including all adult leaders (and yourself!). Group Leaders will receive a copy of the completed forms to their email address. Your Participant List will also indicate submitted forms by name. Links to sign will be valid up to 2 hours depending on form delivery selected.

Child Protection Training
Each adult attending Student Life Camp must complete Lifeway’s Child Protection Training course. Group Leaders will verify all adults have completed this course by signing the Church Agreement (above). This course is the ONLY approved curriculum for Student Life Camp.
Groups attending camp in Texas or New Mexico need to bring physical copies of all completed certificates to be turned in to the location, per state laws.
Participant Lists

You will need your REG ID to access your Participant List. Additionally, if you have more than one REG ID, you will need to enter each one separately to access your Participant Lists. You will not be able to combine lists.
Your completed list is due no later than 14 days before camp.

Southeastern University Swim Permission
In addition to the Student Life Waiver and Release form, Southeastern University requires this Minor Swim Permission Waiver for all minors wanting to use the swimming pool. Please advise parents to enter “Student Life Camp” as the camp name.
YMCA Estes Park
YMCA Chaperone Agreement
Every adult leader in your group will need to read and sign this form. Please print and bring these forms to check in with you for YMCA staff to collect.
Glorieta
Glorieta has their own Waiver. Glorieta will email personalized links to every church group, if you can’t find your link, you can email [email protected] and we will look it up for you. Please include your church name, city and the dates attending.
Liberty University
Rock Wall Assumption of Risk
Each student will need to have a printed and signed form ready to turn in to the rock wall staff prior to using the rock wall.

Cedarville University
Cedarville University Reference Form
Cedarville requests that each adult leader attending camp with a group completes 2 of the reference forms found below, and turns them into the leader of their church group prior to their arrival on campus.
Louisiana Tech
LA Tech Rock Wall Waiver
Any participant interested to using the Rock Wall at Louisiana Tech University is required to submit this online form prior to their arrival at camp.
