Church Agreement

For the Student Pastor or Group Leader to complete and certify all volunteers have passed a background check.

Camp Participant Form

Each person attending Student Life Camp must complete a signed and notarized Participant Form. Follow the steps below to submit your forms. There is no need to bring the originals to camp if we receive them in advance!

  1. Scan all of the Participant Forms (front and back) and send them in one large file to your Event Specialist (Rebekah, Ty or Melissa).
  2. Make the subject line of your email your church name and your camp location.
  3. We will check them off before you arrive to make the check-in process as quick as possible.

Midwestern State University Additional Forms

In addition to the SL Participant Form, please fill out and sign the MWSU Release form. There is one for adult participants and one for minors.

As required by the State of Texas every adult leader attending camp must complete the DSHS Child Protection Training. Please fill out this form as certification upon completion.
