For the Student Pastor or Group Leader to complete and certify all volunteers have passed a background check.
Each person attending Student Life Camp must complete a signed and notarized Participant Form. Follow the steps below to submit your forms. There is no need to bring the originals to camp if we receive them in advance!
Every participant (student and adult) will need to fill out a form the same day you’ll be departing for camp. Please collect these and give to the Student Life staff upon arrival at camp.

Each person attending Student Life Camp must complete a Participant Form, including all adult leaders. The form can be signed electronically and does not require a notary stamp. Group Leaders will receive a copy of the completed forms to their email address.
For 2022 camps, Lifeway’s insurance provider requires us to collect proof of insurance from each church in the form of a Certificate of Insurance (COI) listing your church’s camp dates. Contact your church’s insurance agent and get a copy of the church’s Certificate of Insurance to submit 15 days before camp to CampsCOI@studentlife.com.
Parents will need some key information in order to complete the participant form. Fill-out and provide the following document at your next parent meeting to make completing the participant form a breeze!

COVID-19 Update Student Life Camp

Painting, yard work, landscaping, gardening, trail work or minor construction
Serving in senior care facilities, adult day centers or with children or adults with physical/mental challenges
Helping at a summer kids program or leading a Backyard Kid’s Club
No preference beyond wanting to serve at projects with great need
Sorting, organizing, cleaning for a ministry, nonprofit or school
