Church Agreement

This form certifies all volunteers have passed a background check and have completed Lifeway’s Child Protection Training course (below).

This form will become available to sign via your Participant List link once your final Participant List has been submitted (no later than 35 days before camp). The form will auto-populate the adult leaders’ names that were included on your list (no more typing line by line!). Additional names can be added (even if they are not included on your Participant List) if you need to add backup names in the event you need to replace someone before camp.

Waiver and Release Form

Each person attending Student Life Camp must complete an online Waiver and Release form, including all adult leaders (and yourself!). Group Leaders will receive a copy of the completed forms to their email address. Your Participant List will also indicate submitted forms by name. Links to sign will be valid up to 2 hours depending on form delivery selected.

Child Protection Training

Each adult attending Student Life Camp must complete Lifeway’s Child Protection Training course. Group Leaders will verify all adults have completed this course by signing the Church Agreement (above). This course is the ONLY approved curriculum for Student Life Camp.

Participant List

Your participant list is LIVE and ready for you! Just click the button below to head to our registration landing page to access yours.
You will need your REG ID to access your Participant List. Additionally, if you have more than one REG ID, you will need to enter each one separately to access your Participant Lists. You will not be able to combine lists.
Your completed list is due no later than 35 days before camp.
